Testicular adrenal rest tumour (TART) are a well known complication in adult male patients with CAH. With a reported prevalence up to 94%. In adulthood, the tumours are associated with gonadal dysfunction most probably due to longstanding obstruction of the seminiferous tubules. Because of paucity of studies done to detect TART in pediatric age group and because of importance of early diagnosis and treatment of TART in childhood for fertility protection of males with CAH, this study was applied to detect the presence of TART and its correlation with hormonal control and ther peutic regimen. It is a cross section study in which we studied 30 Egyptian male patients with CAH attended at DEMPU, with duration of the disease equal to or more than 3 years. All patients are on steroid therapy. The study included both patients with 21 hydroxylase deficiency and 11 beta hydroxylase deficiency; the salt wasting and simple virilizing form. The age range was (3.5 to 19 years old) with mean 8.72 years ± 4.48 SD. Our results showed that TART were present in 10% of cases at pediatric age group, it were detected at early childhood (at age 5 years) only after 3 years duration of the disease, it were detected in both SW and SV form of 21 OH deficiency type ofCAH, and it werent felt clinically in any of detected cases. We conclude that routine scrotal ultrasound in early childhood for early diagnosis and management of TART is very important
