  <abstract>To review, study and discuss the role and diagnostic accuracy of multi-detector row CT angiography and color doppler ultrasound studies as minimally invasive and non invasive techniques respectively in the evaluation of patients with carotid artery stenosis. Prospective analysis of stroke related data were obtained from 30 patients (21 Male, 9 Female( and the age ranged from (30-80) years old referred from Neurology department and Internal Medicine department in Cairo University Hospitals for CTA and CDUS examination of the carotid arteries. All patients were examined with doppler indices (PSV, EDV and RI) and different 2D image reconstruction and 3D volume rendering techniques of CTA. The majority of patients were male (70%) with female (30%). The majority of patients 27 patients (90%) had no ulcerated plaques, 3 patients had ulcerated plaques (10%) and the ulcerated plaques were seen by CDUS only. Plaque calcification was seen mainly by CTA in 9 patients (30%), while by CDUS was seen in 3 patients (10%) only. The study showed moderate agreement between both modalities in assessment of the degree of luminal reduction, moderate agreement in assessment of plaque calcification, poor agreement in assessment of plaque ulceration and poor agreement in the assessment of distal run off. Color doppler US is readily available and noninvasive method and without radiation exposure. It allows assessment of both anatomy and hemodynamics of the carotid arteries</abstract>
