This study is a case control study, its aim is to determine whether the carotid arterial wall thickness is a predictor of early atherosclerosis in FMFpatients compared to healthy controls. The study included 45 familial mediterranean fever patients as the cases group and 45 healthy children as the controls group. The study showed that the mean of the carotid intima media thickness is higher in the cases group (0.05) than the control group (0.04) with statistical significant difference. No correlation was found between CIMT and markers of subclinical inflammation as SAA,ESR and CRP.CIMT didn{u2019}t correlate with lipid profile. So carotid intima media thickness may be used as a tool in the prediction of any atherosclerotic burden in children with FMF
