Objectives: To evaluate the role of power Doppler in assessment of synovial inflammation in children with juvenile idiopathic arthritis (JIA) and the role of spectral Doppler resistivity index (RI) in grading of active synovitis in JIA and to correlate semi-quantitative power Doppler score and quantitative spectral Doppler RI index with inflammatory markers and DAS-28. Methods: A cross sectional study was done over a period of 4 months. Thirty one patients of JIA were enrolled. Evidence of arthritis in the form of swelling and tenderness were evaluated and DAS-28 was calculated. A total of 38 clinically swollen knee joints were evaluated by grey scale USG, power Doppler and spectral Doppler to measure RI within blood vessels. Results: Statistically significant correlation between US detected synovial inflammation and resistive index (RI) and between RI and activity parameters (DAS28 and ESR). Conclusions: Power Color Doppler and Spectral Doppler ultrasonography (USG) indices (RI and color fraction) can be used in monitoring disease activity and response to therapy in JIA patients
