Stem cells transplantation (SCT) is a set of techniques aim to replace cells that damaged or destroyed by disease with healthy functioning ones. Although (SCT) is considered as a new life for the patients, it also become very aggressive and causes countless risks on the patients' and their families. Physical, psychological, and social functions of the patients after stem cells transplantation will gradually return to normal within 1 year after transplant. During this recovery period, the transplant patients may have many biopsychosocial complications, such as; physical complications which include pain, fatigue, and signs of rejection, psychological complications such as depression, anxiety and sense of loss and social complications such as social isolation, and inability to return to school or work (Bone marrow & stem cells, 2016). These complications can increase hospital readmission rate to the patients. The health team members have a crucial role in preparing and assisting the patients and their families toward achieving recovery by implementing an individualized discharge plan that should start from first day of admission to insure smooth transfer of transplant patients from hospital to home (Lim, Kenyon, Doorga, & Cave, 2014)
