Introduction: Anemia is one of the main problems that challenges physicians in the pediatric intensive care unit (PICU). One of the main factors contributing to developing anemia in the PICU is blood loss through phlebotomy. Packed red blood cell (PRBC) transfusion remains the main management option of anemia. The real challenges are whether the anemia of the patient is significant and whether the need for PRBC transfusion is the only option the patient has.Aim: To detect the frequency of blood product transfusion in Emergency Room PICU, to find the correlation between phlebotomy losses and PRBC transfusion, and to find the correlation between PRBC transfusion and the duration of mechanical ventilation (MV). Patients and methods: In this prospective observational study conducted in the Emergency room PICU in Cairo University Children hospital, we enrolled 385 pediatric patients during the time period between February and July 2018, 55 of which were excluded, data regarding anemia, phlebotomy loss, PRBC transfusion events, morbidity, and mortality were collected and statistically analyzed
