Background: Post burn hypertrophic scarring represents a challenge to both surgeon and patient. Many surgical modalities and non-surgical modalities were investigated in the management of postburn hypertrophic scarring, Recently the interest in the regenerative properties of fat increased taking advantage of the presence of adipose-derived stem cells. Aim of work:The aim of this study is to compare the effect of fat grafting versus silicon sheet application on post burn hypertrophic scarring Patients and Methods:46 patients suffering from post-burn hypertrophic scars were divided into 2 groups; Group A: 23 patients, the scars were injected with fat Group B: 23 patients used the silicon sheets on the hypertrophic scars. Evaluation was done preoperatively and postoperatively after 3 months through Patient and Observer Scar Assessment Scale (POSAS), caliper and Durometer measurements. Results:Scars treated with autologous fat graft showed statistically significant reduction in hardness measurements in comparison with scars managed with silicon sheets. POSAS scores showed a statistically significant reduction of a great deal of POSAS parameters as a result of an improvement of both clinical evaluation and patient perception
