Background Heart failure is a syndrome characterized by cardiac dysfunction with either left ventricular dilation or hypertrophy causing cardinal manifestations of dyspnea, fatigue and exercise intolerance .Objectives: to determine the effectiveness of Europe heart failure self -care behavioral scale as a new instrument in evaluate quality of care for cardiac patient. Subject: Fifty patients with chronic heart failure selected from Minia university hospital with class III-IV according to New York Heart Association (NYHA) were randomly assigned into two groups equal in number with age range (45-65 years old). Material and Method: the study group was consisted of 25 patients (20 men and 5 women) who participate in a moderate aerobic exercise program (60-70% of maximum heart rate). The session consisted of 5 minutes warming up, 10-15 minutes electronic treadmill program, and 5 minutes cooling down 3 times per weeks for 12 weeks plus received short presentation session for 15 minutes about improvement of self -care behavior one time per week for 12 weeks, in addition to traditional medical treatment, While the control group (20 men and 5 women) received only one presentation session for 15 minutes about improvement of self -care behavior plus traditional medical treatment. Assessment both groups were assessed for The improvement in functional capacity was evaluated by 6MWT, and the improvement in the health related quality of life was measured by MLHFQ and improvement quality of self-care was measured by EHFSCBs at both baseline and following the 12 weeks of exercise and educational management program. Results Highly signicant improvements were noted in the 6MWT by1.25%, improved functional, emotional and psychological status of the patients manifested by a decline in Minnesota Living with Heart Failure Questionnaire physical, psychological and total score by 0.74%, 0.27%, 0.76% respectively and improvement in EHFSCBs after implementing the educational program by 8.5% in the study group compared to the control group
