Purpose and aim of the work: To compare between femtoseconds laser-assisted keratoplasty (FAK) and penetrating keratoplasty (PK) using microscopic examination (light and transmission electron microscopy) of the cut edges of the remaining donor cornea. Material and Methods: Prospective observational study involving 10 eyes; 5 eyes by femto assisted keratoplasty FAK (top hat technique) and the other 5 eyes by conventional trephination method. Main outcomes are histological findings at the cut edge of the donor corneal graft tissue (at 3, 6, 9, and 12 oclock). Results: In femto assisted trephined corneas (FAK), cellular and ultra-cellular changes in the form of marked edema, disruption and disorganization of the collagen fibers, and nuclear changes were more pronounced as compared to conventional PK. In addition, a band of debris with disrupted collagen fibers extending from the anterior to posterior stroma was evident in the corneal stromal layer, compared to very little stromal edema with mechanical trephination
