Bladder cancer is the 9th most common cancer worldwide and the 7th most common worldwide in men. Patient and methods: 65 patients with suspected urinary bladder cancer enrolled in prospective study for local assessment by mp-MRI (multi-parametric MRI) and prediction of muscle invasion via application of VI-RADS (Vesical Imaging-Reporting and Data Systems) scoring system .Results: The ROC (Receiver operating characteristic) analysis demonstrated the optimal criteria for VI-RADS were score >2 with AUC (Area under the curve) of VI-RADS to detect MIBC (Muscle invasive bladder cancer) was 0.98 (95% CI (confidence interval): 0.941), overall accuracy 91.5%, sensitivity and specificity were 97.3% and 81.8 % respectively, PPV (positive predictive value) and NPV (Negative predictive value) were 90% and 94.7% respectively. Conclusion: VI-RADS is a thorough scoring system with perfect sensitivity, specificity, and diagnostic value for assessment of deep muscle invasion in bladder cancer cases.
