Premature ejaculation (PE) is one of the most commonly reported male sexual complaints. The current treatment choice for premature ejaculation is medical treatment. The main limitation of medical treatment for premature ejaculation is recurrence after withdrawal of medication. Aim of the study: To evaluate the Effect of Botulinum-A toxin injection into bulbospongiosus muscle on ejaculation latency time in human. Methods: A total of 25 patients with PE not responding to SSRIs either on demand or daily according to the definition of the forth international consolation of sexual medicine 2018 were involved in this study.Each patient was injected with 20 IU of Botulinum toxin in the bulbospongiosus muscle. evaluation was carried out using intravaginal ejaculation latency time (IELT), Premature diagnostic tool (PEDT), and sexual satisfaction score pre-injection then 1 and 6 months post injection. Results: After one month of treatment, Botox injection in bulbospongiosus muscle increased latency time median among responders median from 1(0.5-1) to 5 (2-20). this increase shows a statistical significance with p value 0.001, Sexual satisfaction score median was significantly improved among responders from 1(1-1) to 3 (1-5), with p value 0.002.Conclusion this pilot study suggest that Botox injection in bulbospongiosus muscle could be effective treatment to improves ejaculatory control and ameliorate couple satisfaction over the sexual intercourse

تعتبر سرعة القذف واحدة من الشكاوى الجنسية الأكثر شيوعا لدى الرجال. إن خيار العلاج الحالي لسرعة القذف هو العلاج الطبي. ولكن يعيب العلاج الطبي تكرار حدوث سرعة القذف بعد الكف عن الدواء.تهدف هذه الدراسة الى تقييم تأثير وسلامة تأثير حقن البوتوكس بالعضلة البصلية الاسفنجية علي سرعة القذف ما مجموعه 25 مريضا بسرعة القذف شاركوا في هذه الدراسة. تم حقنهم بالبوتوكس في العضلة الصلية الاسفنجية، وتم تقييم المرضى عن طريق قياس وقت حدوث القذف والمؤشر العربي لسرعة القذف بعد شهر من الحقن لتقييم الاستجابو مرة اخري بعد 6 شهور لمعرفة مدي الرضا عن النتيجة ةهل حدثت أي مضاعفاتزمتابعة المرضى بعد العلاج اظهرت تحسنا ملحوظا في وقت حودث القذف, تلك الزيادة كانت اعلى بكثير من زمن القذف قبل العلاجز. أن حقن العضلة البصلية الاسفنجية بعقار البوتوكس لهو علاج فعال و امن لعلاج سرعة القذف على المدى الطويل
