Background: Rosacea is a chronic disorder characterized by a variety of clinical manifestations. Four subtypes exist. Rosacea is commonly observed in individuals with Fitzpatrick skin types 1 and 2; Females over the age of 30 are most commonly affected. Ivermectin 1% cream is a topical anti-parasitic agent which has a therapeutic effect due to its activity against demodex mites. PDL have become a mainstay for treating PWS, facial telangiectasias, and rosacea due to their safety and efficacy. Aim of the study: the aim of this study was to provide a comprehensive clinical and dermatoscopic comparative study between the efficacy and safety of pulsed dye laser alone versus its combination with topical ivermectin 1% in the treatment of rosacea.Patients and methods: According to treatment protocol the patients were scheduled to undergo 4 laser sessions with 4 week intervals. Regarding PDL treatment, the laser parameters applied in this study were (5to7 mm spot size, a duration of 450 microsecond and a wavelength of 585 nm, 5 to 6.5 j/cm2). Results: The present study demonstrated the clinical and dermatoscopic effectiveness of both PDL and combined PDL and ivermectin 1% cream for the treatment of rosacea. Both groups showed high score of improvement. Moreover the combined PDL and ivermectin 1% cream was found to have a greater treatment effect than PDL alone, according to the improvement score, and satisfaction score. However, this difference wasn't significant (p= 0.296), (p=0.341) in papular and erythema type respectively
