Dissertation note: Thesis (Ph.D)-Cairo University,2022. Summary: Treatment of completely edentulous patients is primarily done for the sake of restoring function and esthetics. Therefore, patients are always seeking satisfactory results with the least possible postoperative course of complications and pleasant function. This can be perfectly achieved using implant supported restorations. Different types of restorations have been used as a full arch fixed option, however the most commonly used is single unit screw retained restoration. Usually, the abutments used to support this type of restorations are multi-unit abutments. In our study a newly introduced abutment called OT bridge system is used. It overcomes one of the major drawbacks of multiunit abutments which is passivity and the credit goes to the Seeger system used in it. Supporting the restoration on splinted abutments or nonsplinted abutments have been a controversy issue with no definitive consensus yet. In this randomized clinical trial twelve completely edentulous patients (2 females and 10 males) were treated with 48 implants loaded with a temporary acrylic conversion denture. Patients were randomly assigned to be either treated with splinted OT bridge abutments (n=24 implants) supporting a full arch fixed restoration or non-splinted ones (n=24 implants). The marginal bone loss around the implants were evaluated using digital periapical radiographs following the parallel cone technique and analyzed by the Digora system. After 3 months follow-up period a statistically significant difference in marginal bone height existed between baseline at implant placement and after the healing period in both groups. However, on comparing both groups after the healing period there was no statistically significant difference between splinting or unsplinting of the abutments in regards to bone height level. Also, a statistically significant difference existed in each implant between baseline and 3 months follow-up measurements within each group regarding marginal bone loss. Moreover, there was no statistically significant difference existed between the two groups regarding the implant failure in the early phase of healing. In conclusion, both splinting or unsplinting techniques are considered as reliable methods for immediate loading of a full arch screw retained implant restoration as a treatment for completely edentulous patients with no statistically significant difference in marginal bone loss between groups and implant failure, and with significant difference between base line and 3 months in each group at the level of each implant and with adding all of them together.Summary: يتم علاج المرضى الذين يعانون من فقدان كامل للأسنان (عديمي الأسنان) في المقام الأول بهدف استعادة الوظيفة والمظهر الجمالي. ولذلك، يسعى المرضى دائمًا للحصول على نتائج مرضية مع أقل قدر ممكن من المضاعفات بعد العلاج وضمان أداء وظيفي مريح. ويمكن تحقيق ذلك على أكمل وجه باستخدام الترميمات المدعومة بالزرعات السنية. وقد استُخدمت أنواع مختلفة من الترميمات كخيار ثابت لكامل القوس السني، إلا أن أكثرها شيوعًا هو الترميم المكون من وحدة واحدة والمثبت ببراغي. وعادةً ما تكون الدعامات المستخدمة لدعم هذا النوع من الترميمات عبارة عن دعامات متعددة الوحدات (multi-unit abutments). وفي دراستنا هذه، تم استخدام نوع جديد من الدعامات يُعرف بنظام "OT bridge"، والذي يتغلب على إحدى السلبيات الرئيسية للدعامات متعددة الوحدات - وهي مسألة "الخمول" أو الانطباق السلبي (passivity) - وذلك بفضل نظام "Seeger" المستخدم فيه. ولا تزال مسألة دعم الترميمات على دعامات موصولة ببعضها (splinted) أو دعامات غير موصولة (non-splinted) محل جدل، ولم يتم التوصل إلى إجماع قاطع بشأنها بعد. في هذه التجربة السريرية العشوائية، عولج اثنا عشر مريضًا فاقدين لجميع أسنانهم (امرأتان و10 رجال) باستخدام 48 زرعة سنية حُمّلت بطقم أسنان مؤقت من الأكريليك (طقم تحويلي). وقد تم توزيع المرضى عشوائيًا لتلقي العلاج إما باستخدام دعامات "OT bridge" موصولة ببعضها (24 زرعة) لدعم ترميم ثابت لكامل القوس، أو باستخدام دعامات غير موصولة (24 زرعة). وجرى تقييم فقدان العظم الهامشي حول الزرعات باستخدام صور الأشعة السينية الرقمية حول الذروة (periapical radiographs) وفقًا لتقنية المخروط المتوازي، مع تحليل الصور باستخدام نظام "Digora". وبعد فترة متابعة استمرت 3 أشهر، لوحظ وجود فرق ذي دلالة إحصائية في مستوى العظم الهامشي بين القياسات الأساسية عند وضع الزرعة والقياسات بعد فترة الالتئام في كلتا المجموعتين. ومع ذلك، عند مقارنة المجموعتين بعد فترة الالتئام، لم يظهر فرق ذو دلالة إحصائية بين استخدام الدعامات الموصولة أو غير الموصولة فيما يتعلق بمستوى ارتفاع العظم. كذلك، وُجد فرق ذو دلالة إحصائية لكل زرعة عند المقارنة بين القياسات الأساسية وقياسات المتابعة بعد 3 أشهر داخل كل مجموعة فيما يخص فقدان العظم الهامشي. وعلاوة على ذلك، لم يكن هناك فرق ذو دلالة إحصائية بين المجموعتين فيما يتعلق بفشل الزرعات خلال المرحلة المبكرة من الالتئام. ختاماً، تُعد كل من تقنيتي الربط (splinting) وعدم الربط (unsplinting) وسيلتين موثوقتين للتحميل الفوري للتركيبات السنية المثبتة ببراغي على الغرسات لتغطية القوس السنية الكاملة لدى المرضى فاقدي الأسنان تماماً؛ إذ لم يُلاحظ وجود فرق ذي دلالة إحصائية بين المجموعتين فيما يتعلق بفقدان العظم الحافي أو فشل الغرسات، في حين وُجد فرق معتد به إحصائياً عند مقارنة القياسات الأولية (خط الأساس) بتلك المسجلة بعد ثلاثة أشهر في كل مجموعة، سواء على مستوى كل غرسة على حدة أو عند تجميع بيانات الغرسات كافة.
