Summary: It is a study on 52 patients with traumatic sub-axial cervical facet sub-laxation or dislocation managed surgically via: anterior/posterior/combined approaches. Adequate decompression of the neural elements and the restoration of sufficient spinal stability is the aim. All approaches are safe & sufficient in reduction & fixation with very close clinical & radiological successful rates. Factors to be considered in treatment planning include neurologic status, degree and type of bony and/or ligamentous disruption, and degree of spinal cord compression. Summary: تضمن هذا البحث دراسة على 52 مريضاً قد تعرضوا لإصابات أدت لحدوث خلع بوجيهات مفاصل الفقرات العنقية تحت المحورية ، وتم إجراء الجراحات لهؤلاء المرضى في الفترة بين يوليو 2021 وديسمبر 2023 . مثل هذه الإصابات تشكل حوالي (45%) من إصابات الفقرات العنقية وتنتج عادة عن الانثناء للأمام مع الدوران أو الانثناء للأمام مع التباعد بين الفقرات العنقية ، هذه الآلية تجعل الوجيه المفصلي الأسفل للفقرة العلوية ينزلق إلى الأمام على الوجيه المفصلي العلوي للفقرة السفلية ، وهذا يمكن أن يحدث على جانب واحد أوعلى الجانبين . هذا النوع من الإصابات هو من الإصابات غير المستقرة لأنه يكون مصحوبا باضطراب الأربطة البين والفوق شوكية والرباط الأصفر والرباطين الطوليين الأمامي والخلفي والغضاريف بين الفقرات ، مما قد يؤدي إلى الضغط على الحبل والشوكي والجذور العصبية وضيق القناة الشوكية والثقب العصبية . كانت النسبة الأكبر للحالات من الذكور وشكلت حوادث السير النسبة الأكبر من الإصابات.
