Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Physical Therapy - Department of Growth and Development Disorsers Pediatrics Summary: Background: Patient registry is 2an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves a predetermined scientific, clinical, or policy purpose(s). Aim; this study was conducted to establish data base for cerebral palsy (CP) in Qalubia governorate. Subjects and Methods: children with CP who are receiving physical therapy services in Al Nile insurance hospital, Benha University hospital, Shibin el kanater governmental hospital, Toch governmental hospital, Khanka governmental hospital, Kafr shokr and Qalub privet centers, that was conducted from February till August 2018,from both genders, ranged in age from 2 months to 18years . The number of cases which included in this study was 181 CP cases out from 226 cases, 94 cases male and 87 cases female. The outcomes measures were Viking speech Scale (VSS), Manual Ability Classification System (MACS), Gross Motor Function Classification System (GMFCS) and Gross Motor Function Measure (GMFM)
