Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Cardiology Summary: Background: Inflammation andoxidative stress have played important roles in all stages of the evolution of atherosclerotic plaque, from the early development of endothelial dysfunction.(1) Myeloperoxidase participates in proatherogenic biological activities related to the evolution of cardiovascular disease, including initiation, propagation and acute complications of atherosclerotic process.(2) In this study we identified the relationship between myeloperoxidase (MPO) and the severity of coronary artery disease (CAD). Methods: we measured the level of MPO in the peripheral venous circulation and the coronary arteries in patients with CAD and compared them to the levels of MPO in a control group who don{u2019}t have CAD. The severity of CAD was quantitated using the CASS (vessel) score and the Gensini score. Results: This study was performed on 86 patients (divided into case and control groups) recruited over 3 months duration in the cardiovascular department of Cairo university hospital. We found a strong correlation between serum MPO and severity of coronary artery disease as measured using the Vessel (CASS) score (p=0.05) and the Gensini score. (p=0.002). We also have found that the peripheral venous serum level of MPO correlates with the intracoronary arterial serum levels of MPO and the differences were not statistically significant. Conclusions: There is a Correlation between the serum MPO and the severity of coronary artery disease. The peripheral venous serum level of MPO reflects the intracoronary arterial serum levels of MPO
