520			_aAim of work Assess the prognostic value of liver dysfunction indices; the Albumin/Bilirubin (ALBI) score, the Model for End-Stage Liver Disease Excluding INR (MELD-XI) score, the Model for End-Stage Liver Disease including Albumin (MELD-Alb) score, liver fibrosis 4 (FIB-4) index, and live fibrosis 5 (FIB-5) index on predicting the in-hospital mortality of patients admitted with acute heart failure (AHF). Methods The ALBI, MELD-XI, MELD-Alb, FIB-4, FIB-5 scores as well as Get With The Guidelines Heart Failure (GWTG-HF) risk score were calculated at admission for a total of 107 patients admitted with AHF who were prospectively followed for all-cause in-hospital mortality. Each score was statistically analyzed as a continuous and categorial (three tertiles) variable. Logistic regression was done to detect independent predictors of mortality. Receiver-operating characteristic curve (ROC) curve was constructed with area under curve analysis performed to detect best cutoff value and compare the prognostic accuracy of the scores for the prediction of mortality. Furthermore, multivariate logistic regression analysis using the significant scores was done to assess if combining two of them is of additive predictive significance. P-values less than 0.05 were considered as statistically significant.
520			_aيرتبط الفشل القلبي الحاد بمعدلات عالية من الاعتلال والوفيات ، و بالتالي من الضروري تصنيف المرضى للتعرف علي المرضى الأكثر عرضة لمخاطر المرض. لا يؤثر الفشل القلبي الحاد علي القلب فقط بل يصل تأثيره الي جميع أعضاء الجسم بما فيهم الكبد لما يسببه من نقص لتدفق الدم و الاحتقان الوريدي العام مما يؤدي الي اختلال وظائف الكبد. وقد اثبتت العديد من الدراسات ارتباط الاعتلال الكبدي بارتفاع معدل الوفيات ضمن مرضي الفشل القلبي الحاد. و قد قامت دراستنا بتقييم دور دلالات الاعتلال الكبدي التالية (ALBI, MELD-XI (FIB-5, FIB-4, MELD-Alb, والمقارنة فيما بينها لأول مره (على حد علمنا) في التنبؤ بالوفيات داخل المستشفى لمرضي الفشل القلبي الحاد. ضمت الدراسة 107 من مرضى الفشل القلبي الحاد الذين قد تم حجزهم بوحدة الرعاية المركزة الخاصة بقسم الأمراض الباطنة بكلية طب القصر العيني – جامعة القاهرة. حيث تم تسجيل بيانات المرضي متضمنة السن و الجنس و العلامات الحيوية ،وعمل الفحوصات المعملية لدلالات وظائف الكبد و الكلي، و الفحوصات السريرية متصمنة الموجات فوق الصوتية للقلب، وتم احتساب دلالات الخطورة عند الدخول للمستشفي والتي تضمنت (GWTG-HF) و دلالات الاعتلال الكبدي ( ALBI, MELD-XI (FIB-5, FIB-4, MELD-Alb, ومن ثم تم متابعة المرضي حتي تحسن حالتهم الصحية و الخروج من المستشفى او حدوث الوفاة، حتي يتثني لنا تقييم دور هذه الدلالات في التنبؤ بحدوث الوفيات داخل المستشفى.
