520		3	_abackground This study was designed to compare between perineural versus intravenous dexamethasone in prolonging the analgesic effect of supraclavicular plexus nerve block in upper limb surgeries Patients and methods Forty-four patients who underwent upper limb surgeries at the national cancer institute, Cairo University were included in this prospective, randomized, and controlled study .They were randomly assigned before surgery between two groups, with twenty-two patients in each. Differences in analgesic duration between the two groups,; Group A (PerineuralDexamesthasone group):The patients in those cases were administered with 2ml of dexamethasone (8mg) combined with 28 ml of bupivacaine 0.25% concentration perineurally, along with 2ml of normal saline as an IV placebo. Group B (Systemic dexamethasone group): In these cases, a 2ml volume of normal saline was combined with 28 ml of bupivacaine 0.25% concentration perineurally, and a 2ml volume of dexamethasone (8mg) was administered intravenously as a systemic adjuvant. Blood pressure, heart rate, were assessed every 5 minutes intraoperatively., visual analogue scale score were assessed immediately postoperative every hour for the first 10 h and then two hourly till 24 h after end of surgery. VAS is a 0 -10 scale in which 0 means that the patient feels no pain and 10 means that the patient is in maximal pain. Morphine sulfate 0.03 mg/kg was used for postoperative analgesia on demand (VAS ≥4) with maximum Dose of 2 mg/kg per day. Results: This study showed that When compared to its intravenous counterpart, perineural dexamethasone (8 mg) offers a prolonged duration of motor block, sensory block, and postoperative analgesia in ultrasound-guided supraclavicular brachial plexus block.
520		3	_aالخلفية العلمية و الهدف من الرساله: ان هدف الدراسة الحالية هو مقارنة تأثيرات الديكساميثازون حول الأعصاب والديكساميثازون الوريدي في مدة التحليل بعد الجراحة لدى المرضى البالغين الذين يخضعون لعمليات جراحية اختيارية لعصب الضفيرة فوق الترقوة في جراحات الأطراف العلوية تحت كتلة الضفيرة العضدية فوق الترقوة. طرق البحث : تم إجراء هذه الدراسة العشوائية مزدوجة التعمية على 44 مشاركًا. تم تقسيمالمرضي إلى مجموعتين متساويتين: المجموعة (أ): تلقت "المجموعة المحيطة بالعصب" كتلة عصبية تحتوي على 28 مل من بوبيفاكايين 0.25% و2 مل من الديكساميثازون (8 ملغ) كمساعد حول الأعصاب و2 مل من المحلول الملحي العادي كعلاج وهمي وريدياً. المجموعة (ب): تلقت "المجموعة الوريدية" 28 مل 0.25% بوبيفاكايين و2 مل من محلول ملحي عادي كمساعدحولالأعصاب و2 مل ديكساميثازون (8 ملغ) عن طريق الوريد. النتائج: عند مقارنته بنظيره الوريدي، يوفر ديكساميثازون (8 ملغ) حول العصب مدة طويلة من الإحصار الحركي والإحصار الحسي وتسكين الألم بعد العملية الجراحية في إحصار الضفيرة العضدية فوق الترقوة الموجه بالموجات فوق الصوتية.
