520		3	_aAim: To determine and compare the clinical and cost-effectiveness of Allium Sativum oil (ASO) versus Mineral trioxide aggregate (MTA) in primary molars pulpotomy in a group of Egyptian children. Methodology: This is a randomised clinical pilot study with two arms, parallel groups, and allocation blinded. Twenty-six children participated in this clinical trial with fifty-two teeth (n=52) maxillary and mandibular second primary molars indicated for vital pulpotomy and randomly allocated into two equal groups(n=26). Participant ages ranged from 5 to 7 years. Group (I): pulpotomy using Allium Sativum Oil, and Group (II): using Mineral trioxide aggregate as pulpotomy agents. The follow-up period was one year, and the primary outcome was evaluating clinical effectiveness for both groups. The clinical and radiographic assessment was done with the help of predetermined criteria during the follow-up period and according to the participant timeline. The secondary outcome was cost-effectiveness, measured using incremental cost-effectiveness ratio (ICER) and average cost-effectiveness ratio (ACER). Results: The relative risk of radiographic failure signs of the ASO group over the MTA group was (1.3) (95% CI 0.89:1.7) after 12 months. MTA had a higher (ACER) (67.00) than ASO (39.55). Conclusion: Allium Sativum oil is non-inferior to Mineral trioxide aggregate regarding clinical efficacy. ASO has a lower average cost-effectiveness ratio (ACER) than MTA.
520		3	_aان الهدف الأساسي من هذه الدراسة هو التحقق من الفعالية الإكلينيكية والفعالية من حيث التكلفة لزيت الثوم كمادة لبضع اللب مقارنة بمجموع حصى ثلاثي أكسيد المعادن في الأضراس الأولية في مجموعة من الأطفال المصريين. كانت الدراسة عبارة عن تجربة عشوائية مزدوجة التعمية والتخصيص، ذراعان، ومجموعات متوازية. التوزيع العشوائي المتساوي بتصميم مجموعات متوازية كان بنسبة تخصيص ١:١. التجربة باستخدام الحاسوب لتعيين السن الذي سيكون إما في مجموعة التحكم أو مجموعة التدخل. جريت هذه الدراسة في قسم طب أسنان الأطفال -كلية طب الفم والأسنان -جامعة القاهرة -مصر. كانت هذه تجربة سريرية عشوائية مزدوجة التعمية ذات ذراعين ومجموعتين متوازيتين وتخصيص أعمى. كان العدد الإجمالي للمشاركين ٢٦، تتراوح أعمارهم بين ٥-٧ سنوات في الاضراس الاولية الثانية المنخورة غير المصحوبة بأعراض. كانت فترة المتابعة سنة واحدة، وكانت زيارات متابعة التقييم الشعاعي في ٦ و١٢ شهرًا. كانت حصيلة النتائج الفعالية السريرية والفعالية من حيث التكلفة. في هذه الدراسة، كان المشاركون في مجموعة زيت الثوم ٥٠٪ من الذكور والاناث بينما في مجموعة حصى ثلاثي أكسيد المعادن، كان هناك ٦٥٪ من الذكور و٣٥٪ من الاناث. كان متوسط الاعمار في مجموعة زيت الثوم (٠.٦٧ ± ٦.٥٨) سنوات وفي مجموعة حصى ثلاثي اكسيد المعادن (٠.٩٥±٦.٢٢) سنوات. لم يكن هناك فرق كبير بين المجموعات المختبرة فيما يتعلق بالجنس (p= 0.523) والعمر (p=0.174). أظهرت النتائج أن زيت الثوم لم يكن أدنى من الناحية الإحصائية من حصى ثلاثي أكسيد المعادن في 12 شهرًا. كان حصى ثلاثي أكسيد المعادن أعلى من زيت الثوم في متوسط النسبة الفعالة من حيث التكلفة. زيت الثوم مادة واعدة لبتر اللب بالإضافة إلى تكلفتها المنخفضة وهي إجراء أقل حساسية من الناحية التقنية مقارنةً بـحصى ثلاثي أكسيد المعادن، يوصى باستخدامها كمادة لبتر اللب عندما تكون التكلفة مصدر قلق في بعض الظروف الاقتصادية.
