520		3	_aBackground: The inflammatory disorder known as psoriatic arthritis (PsA) lasts a long time and affects the joints in 20-30% of individuals with psoriasis (Pso). It causes pain, stiffness, swelling, and joint damage. Pulsed high-intensity laser therapy (PHILT) has shown the potential effect of reducing inflammation, relieving pain, improving mobility, and enhancing function. Purpose: This study was conducted to assess the efficacy of PHILT on psoriatic hand arthritis (PsHA). Subjects and methods: 76 patients of both genders, aged 30-50, with asymmetric PsHA were recruited from the Suez Canal authority hospital in Ismailia Governorate and divided equally into two groups at random. The study group (Group A, n=38) underwent PHILT alongside a conventional physical therapy program that included (heat therapy, stretching, and strengthening exercises). The control group (Group B, n=38) received a sham PHILT along with the same physical therapy regimen. Treatment continued three sessions a week for eight consecutive weeks for each group. Variables were measured pre and post-intervention, including handgrip strength (HGS), joint swelling, tenderness counts, and the visual analogue scale (VAS) scores. Results: After the 8-week treatment period, group (A) exhibited significant improvements in outcome measures compared to group (B). The improvement percent in group (A) was 33.17% for HGS, 53.72% for joint swelling count, 61.14% for joint tenderness count, and 57.43% for VAS, while in group (B) was, 17.02%, 25.87%, 24.93%, and 32.38% respectively when resembled to their levels before therapy (p < 0.01). Conclusions: Counting PHILT into routine physical therapy may yield better results for patients with PsHA than traditional treatment alone.
520		3	_aأجريت هذه الدراسة لتقييم فاعلية العلاج بالليزر النابض عالي الشدة في علاج التهابات مفاصل اليد بمرضي الصدفية على 76 مريضًا (24 مريض و52 مريضة) يعانون من التهاب مفاصل اليد نتيجة اصابتهم بمرض الصدفية، تراوحت أعمارهم بين 30 إلى 50 عامًا. تم اختيار المرضى من العيادة الخارجية للعلاج الطبيعي بمستشفى هيئة قناة السويس بمحافظة الإسماعيلية وتم تقسيمهم عشوائيًا إلى مجموعتين متساويتين في العدد: كل مجموعة تتكون من 38 مريضًا. المجموعة (أ): (مجموعة الدراسة): تلقت الليزر النابض عالي الشدة (10 دقائق للجلسة الواحدة) بالإضافة الي برنامج العلاج الطبيعي التقليدي (30 دقيقة للجلسة الواحدة والتي تنقسم الي 10 دقائق علاج مائي دافئ، 10 دقائق تمرينات استطالة لعضلات ثني اليد المهيمنة، 10 دقائق لعمل تمرينات تقوية لعضلات ثني اليد المهيمنة)، المجموعة (ب): (المجموعة الضابطة): تلقت فقط نفس برنامج العلاج الطبيعي التقليدي لمجموعة الدراسة، استمر العلاج لمدة 8 أسابيع متتالية بمعدل 3 جلسات أسبوعيا لكلا المجموعتين (أ، ب). تم تقييم قوة قبضة (بواسطة مقياس القوة اليدوي) والم (بواسطة مقياس التناظر البصري) وتورم والتهاب مفاصل اليد المهيمنة قبل وبعد العلاج. وقد أظهرت النتائج أن هناك زيادة ذو دلالة إحصائية في متوسط قيمة قياسات قوة قبضة اليد عند المقارنة بين قبل وبعد العلاج في المجموعتين (أ، ب) ولكن كان الفارق أكبر في المجموعة (أ)، انخفاض ذو دلالة إحصائية في متوسط قيمة قياسات تورم والتهاب والام مفاصل اليد عند المقارنة بين قبل وبعد العلاج في المجموعتين (أ، ب) ولكن كان الفارق أكبر في المجموعة (أ)، وبعد مناقشة هذه النتائج وتحليلها، تبين أن تقنيه العلاج بالليزر النابض عالي الشدة له الأفضلية في تحسين التهاب مفاصل اليد لمرضي الصدفية فيما يخص زيادة قوة قبضة اليد وانخفاض التورم والالتهاب والألم بها.
