520		3	_aBackground and Aim: Nuclear unrest is a term applied to Wilms tumors (WT) with favorable histology that show nuclear enlargement similar to that seen in anaplasia but with no abnormal mitotic figures. This study aimed to find out the impact of nuclear unrest grades on unilateral favorable histology Wilms tumor (FHWT). Patients and Methods: This retrospective study included 62 patients with unilateral favorable histology Wilms tumor at National Cancer Institute-Egypt (median age 40 months) treated from January 2008 to December 2017. Results: Among 62 patients with pathologically confirmed FHWT, 17(27.4%) patients were found to have no nuclear unrest (grade 0), 13 (21%) patients had grade I, 13 (21%) patients had grade II and 19 (30.6%) patients had grade III nuclear unrest. Nuclear unrest grade 3 were more common in patient’s age ≥5 years (p=0.03), and predominated in patients who underwent upfront nephrectomy (p=0.01), however, gender, stage distribution and response to treatment were similar to that for WT with other grades of nuclear unrest. Estimated 4-year event-free survival (EFS) and overall survival (OS) of the study were 81.8%±5% and 86.4% ±4.5%. Nuclear unrest grade 2 and 3 grouped together had statically significant lower EFS versus grade 0 and 1, 71%, 93.3% respectively (p = 0.029). multivariate analysis revealed that metastatic disease and nuclear unrest grade 2 -3 were an independent prognostic factor for EFS. p= 0.003 with HR 7.2 (95.0% CI :1.5-34.1) for metastatic disease and P= 0.013 with HR 6.8 (95.0% CI: 1.9 -23.8) for nuclear unrest grade2-3. Metastatic disease at diagnosis was prognostic for OS (p =<0.001). Conclusions: Metastatic disease at diagnosis clearly remains the most important prognostic factor in unilateral FHWT. Nuclear unrest grade 2 and 3(grouped together) adversely affect EFS in unilateral FHWT. Larger analysis will be necessary to consolidate our results. Ultimately, international harmonization and clarification of definition of nuclear unrest grades may help precise grading and consequently, treatment modification for better survival.
520		3	_aالاضطرابات النووية هو مصطلح ينطبق على أورام ويلمز ذات الأنسجة المواتية التي تظهر تضخمًا نوويًا مشابهًا لذلك الذي يظهر في فقدان التنسج ولكن مع عدم وجود أرقام انقسامية غير طبيعية. هدفت هذه الدراسة إلى معرفة تأثير درجات الاضطرابات النووية على الأنسجة المواتية لورم ويلمز, تم اجراء هذه الدراسة علي عدد 62 طفلا مصابين باورام الكلي علي احد الجانبين والذين تم تشخيصهم طبقا لنتائج تحليل الانسجة (خاصة نتائج الأنسجة المفضلة) وذلك بقسم اروام الأطفال بالمعهد القومي للسرطان في مصر في الفترة خلال الفترة من يناير 2008 إلى ديسمبر 2017بالاضافة الي تقييم المرضى لمعرفة تأثير الاضطرابات النووية الخاصة بالخلايا بالنسبة البقاء على قيد الحياة بصفة عامة و البقاء علي قيد الحياة من بداية العلاج و لحين حدوث تطور وعلاقتهم بعوامل تطور المرض .
