520		3	_aBackground: Hearing difficulties (HD) in adults who have normal thresholds on pure tone audiometry (PTA) is a common condition with a prevalence of 5%-37% in patients complaining of hearing problems in an audiology clinic but also commonly ignored. There are many possible causes including hidden hearing loss (HHL) or cochlear synaptopathy, which is a perceptual disorder characterized by complaining of hearing difficulties despite falling within normal audiometric thresholds, especially in challenging listening conditions as noisy environments. Objectives: To investigate patients complaining of hearing difficulties despite normal audiogram thresholds using SPIN test, ABR and OAEs. To explore whether these tests could serve as diagnostic tools for hidden hearing loss. Methods: A case control study, each of the study and the control groups comprised 40 subjects; age and sex matched. Age ranged from 18 to 50 years. Hearing was assessed in both groups using PTA, SPIN test, ABR and OAEs. Results: SPIN test findings showed a statistically significant difference between the study and the control groups. Also, ABR showed a statistically significant differences regarding amplitude of wave I and V/I ratio between the study and the control groups in both ears. Both ears showed a statistically significant positive correlation between SPIN score and ABR wave I amplitude. Conclusion: Patients who complain of hearing difficulties with normal audiogram showed poorer SPIN test scores, a reduction in ABR wave I amplitude and elevation in V/I ratio in the current study.
