520		3	_aBackground. As a recent addition to pigmentation therapy, tranexamic acid (TA) is receiving increasing attention for the treatment of melasma. Fractional lasers and microneedling have recently emerged as innovative drug delivery techniques aimed at enhancing the efficacy of topical treatments. Objective. To compare the efficacy and safety of fractional 2940‑nm erbium: yag (Er: YAG) laser with microneedling for trans epidermal delivery of TA in patients with melasma. Methods and materials. Thirty postpartum female patients with bilateral, symmetrical facial melasma were randomly assigned to receive fractional Er: YAG laser treatment on one side of the face and microneedling on the other, each flowed by topical application of 5% TA solution. The treatment protocol consisted of five biweekly sessions. Assessment included the melasma area and severity index (MASI) score, physician’s evaluation, patient satisfaction, and dermoscopy. Results. Three months after the last session, both the laser and microneedling were significantly effective for the delivery of TA. However, the fractional Er: YAG laser resulted in significantly better clinical and dermoscopic improvement compared to microneedling. Post‑inflammatory hyperpigmentation (PIH) was observed on the laser‑treated sides, whereas higher pain scores were reported on the microneedling‑treated sides. Conclusion. Both fractional Er: YAG laser and microneedling can be successfully used for the delivery of TA in the treatment of postpartum melasma.
520		3	_aالغرض من هذه الدراسة هو تقييم فعالية استخدام ليزر الإربيوم ياج المجزأ و تقنية الوخز بالإبر الدقيقة في توصيل حمض الترانيكسامك في علاج الكلف بعد الولادة. وقد ضمت 30 مريضه وتم وضع معايير صارمة لإدراج المرضى للدراسة او ستبعادهم منها ، حيث تم علاج من شملتهم الدراسة باستخدام كل من ليزر الإربيوم ياج المجزأ في توصيل حمض الترانيكسامك على جانب من الوجه وتقنية وخز الإبر الدقيقة في توصيل حمض الترانيكسامك على الجانب الآخر، وقد تلقى جميع المرضى خمسة جلسات بفاصل أسبوعين بين الجلسات وتم إجراء المتابعة التقييمية لفاعلية العلاج بعد ثلاثة أشهر من انتهاء الجلسات وبتحليل نتائج علاج الكلف اتضح ان علاج الكلف بالأسلوب المشترك المكون من ليزر الاربيوم ياج المجزأ وحمض الترانيكسامك قد أدى إلى نقص مؤثر في مساحة المنطقة المصابة وكانت النتائج افضل من الأسلوب المشترك المكون من تقنية وخز الابر الدقيقة وحمض الترانيكسامك. وتشير نتائج البحث ان الأسلوب المشترك المكون من ليزر الاربيوم ياج المجزأ وحمض الترانيكسامك أدى إلى آثار جانبية اقل من حيث الشعور بالألم أثناء الجلسة بينما أدى إلى أثار جانبية اعلى من حيث فرط التصبغ بعد الالتهاب بعد الجلسات حيث تم تسجيل ٣ حالات بينما لم يتم تسجيل اى حالة فرط تصبغ بعد الالتهاب بعد الجلسات باستخدام الأسلوب المشترك المكون من تقنية الوخز بالابر الدقيقة وحمض الترانيكسامك. وخلصت الدراسة ان الأسلوب العلاج المشترك من ليزر الاربيوم ياج المجزأ وحمض الترانيكسامك يمكن أن يكون علاجا مؤثرا لهذا المرض العنيد وأظهر تفوقاً ملحوظاً في النتائج وهو افضل من الأسلوب المشترك من تقنية الوخز بالابر الدقيقة وحمض الترانيكسامك.
