520		3	_aPurpose: to evaluate the efficacy of the of low-level laser versus resisted exercise in hand function of patients with systemic sclerosis. Methods of evaluation: Measurement of the skin thickness via the modified Rodnon skin score (RSS), serum cortisol level (SCL) and Hand Grip Dynamometer (HGD) . Methods:- Forty patients who have hand involvement of patients with scleroderma, were participated in the study. They recruited from the dermatology departments of Cairo university hospitals and Om El-Misrieen hospital. Their ages were ranged from 45 to 60 years. They were randomly divided into two groups in number, one group (A) and a group (B). Group A: This group included 20 patients who had scleroderma in dorsum of hand and received low level laser and traditional physical therapy (stretching, and active exercise). patients received 3 sessions per week for 12 weeks (3 months), time of session is 20 minutes. Group B: This group included 20 patients who had scleroderma in the dorsum of hands and who received resisted exercises and traditional physical therapy (stretching and active exercise). patients received 3 sessions per week for 12 weeks (3 months), time of session is 20 minutes . Results and conclusion:- Results showed a highly significant reduction in RSS and SCL with a highly significant increase HGD at the end of the treatment program in groups (A) and (B) . So both the low level laser with traditional physical therapy and resisted exercises with traditional physical therapy were equivalent in improving hand function in patients with systemic sclerosis as manifested by the highly decreases of RSS, SCL and highly increases of HGD.
520		3	_aالهدف: تقييم فعالية العلاج بالليزر منخفض المستوى مقابل تمارين المقاومة في تحسين وظائف اليد لدى مرضى التصلب الجهازي. طرق التقييم: قياس سماكة الجلد باستخدام مقياس رودنون المعدل لسمك الجلد (RSS)، ومستوى الكورتيزول في الدم (SCL)، وقوة قبضة اليد (HGD). المنهجية: شارك في الدراسة أربعون مريضًا يعانون من مشاكل في اليد نتيجة التصلب الجهازي. تم اختيارهم من أقسام الأمراض الجلدية في مستشفيات جامعة القاهرة ومستشفى أم المصريين. تراوحت أعمارهم بين 45 و60 عامًا. تم تقسيمهم عشوائيًا إلى مجموعتين، المجموعة (أ) والمجموعة (ب). المجموعة (أ): ضمت هذه المجموعة 20 مريضًا يعانون من التصلب الجهازي في ظهر اليد، وتلقوا العلاج بالليزر منخفض المستوى والعلاج الطبيعي التقليدي (تمارين التمدد والتمارين النشطة). تلقى المرضى 3 جلسات أسبوعيًا لمدة 12 أسبوعًا (3 أشهر)، مدة الجلسة 20 دقيقة. المجموعة ب: شملت هذه المجموعة 20 مريضًا مصابًا بتصلب الجلد في ظهر اليدين، وتلقوا تمارين مقاومة و علاجًا طبيعيًا تقليديًا (تمارين إطالة وتمارين نشطة). تلقى المرضى 3 جلسات أسبوعيًا لمدة 12 أسبوعًا (3 أشهر)، مدة الجلسة 20 دقيقة. النتائج والاستنتاج: أظهرت النتائج انخفاضًا ملحوظًا في كل من مؤشر شدة التصلب (RSS) ومؤشر شدة التصلب (SCL)، مع زيادة ملحوظة في مؤشر تباين اليد (HGD) في نهاية برنامج العلاج في المجموعتين (أ) و(ب). وبالتالي، كان كل من الليزر منخفض المستوى مع العلاج الطبيعي التقليدي، وتمارين المقاومة مع العلاج الطبيعي التقليدي، متكافئين في تحسين وظيفة اليد لدى مرضى التصلب الجهازي، كما يتضح من الانخفاضات الملحوظة في مؤشر شدة التصلب (RSS) ومؤشر شدة التصلب (SCL)، والزيادات الملحوظة في مؤشر تباين اليد (HGD).
