520		3	_aPurpose: To evaluate the efficacy of Laser therapy and therabite exercises on pain and mandibular trismus after head and neck Caner surgeries. Methods: Sixty patients with pain and trismus after head and neck cancer surgeries receiving radiotherapy were recruited from out–patient clinics of National Cancer Institute, Cairo University and were divided randomly to three groups. Group (A) composed of 20 patients received Laser therapy plus the conventional exercise therapy (gentle stretches, resisted exercises and passive range of motion exercise) for three months. Group (B) received the therabite exercises plus the same conventional exercise therapy for three months also. Group (C) composed of 20 patients received Laser therapy and TheraBite exercises plus the conventional exercise therapy (gentle stretches, resisted exercises and passive range of motion exercise) for three months. Methods of evaluation: (Measurement of the maximal interincisal opening (MIO), mandibular Function Impairment questionnaire (MFIQ) and visual analogue scale (VAS) were measured before and after treatment . Results and conclusion: – Results showed that application of Laser therapy and therabite exercises with the conventional exercise therapy had a valuable effect on pain and mandibular trismus after head and neck surgery in cancer patients as evidenced by the highly significant increases in MIO and the highly significant decreases in MFIQ and (VAS).
520		3	_aالغرض:يهدف هذا البحث إلى تقييم فعالية العلاج بالليزر وتمارين جهاز "ثيرابايت" في تخفيف الألم وتحسين حالة تقييد فتح الفك السفلي (التريسماس) بعد جراحات الرأس والرقبة لدى مرضى السرطان. طرق التقييم: تمت عملية التقييم من خلال قياس المسافة العظمى لفتح الفم بين القواطع العلوية والسفلية (Maximal Interincisal Opening - MIO)، واستبيان تقييم قصور وظيفة الفك السفلي (Mandibular Function Impairment Questionnaire - MFIQ)، ومقياس التقدير البصري للألم (Visual Analogue Scale - VAS). المنهجية:شارك في الدراسة ستون مريضًا مصابين بتقييد فتح الفك (تريسماس) بعد إجراء جراحات الرأس والرقبة، وقد تم اختيارهم عشوائيًا من العيادات الخارجية للمعهد القومي للأورام – جامعة القاهرة، بواقع 30 ذكرًا و30 أنثى، جميعهم من مرضى السرطان الذين تلقوا العلاج الإشعاعي، وتتراوح أعمارهم بين 40 و60 عامًا. تم تقسيمهم إلى ثلاث مجموعات متساوية:المجموعة (أ): 20 مريضًا تلقوا العلاج بالليزر بالإضافة إلى برنامج العلاج التمريني التقليدي (تمددات لطيفة، تمارين مقاومة، وتمارين نطاق الحركة السلبية) لمدة ثلاثة أشهر، المجموعة (ب): 20 مريضًا تلقوا تمارين جهاز "ثيرابايت" بالإضافة إلى نفس برنامج العلاج التمريني التقليدي لمدة ثلاثة أشهر، المجموعة (ج): 20 مريضًا تلقوا العلاج بالليزر وتمارين جهاز "ثيرابايت" معًا بالإضافة إلى برنامج العلاج التمريني التقليدي. النتائج والخلاصة:أظهرت النتائج أن استخدام العلاج بالليزر وتمارين جهاز "ثيرابايت" مع برنامج العلاج التمريني التقليدي كان له تأثير ملحوظ في تخفيف الألم وتحسين قدرة فتح الفك السفلي بعد جراحات الرأس والرقبة لدى مرضى السرطان الذين تلقوا العلاج الإشعاعي، وذلك من خلال الزيادة الكبيرة والدالة إحصائيًا في قيمة MIO، والانخفاض الكبير والدال إحصائيًا في درجات MFIQ وVAS.
