520		3	_aWhen attempting to place an intrauterine device (IUD) into a narrow cervical canal, technical difficulties may occur, and women may experience pain (Balica et al., 2018). IUD insertion-related difficulties and adverse effects are more likely among women who have never given birth vaginally, according to a few studies, and many healthcare providers do not counsel or prescribe IUDs to nulliparous women (Ashour et al., 2020). An American College of Obstetricians and Gynecologists committee opinion advises practitioners to utilise IUDs as a first line of contraception in nulliparous women, dispelling this misconception and confirming IUD safety and efficacy in this population of patients (Espey et al., 2019). Misoprostol is a prostaglandin E1 analogue approved for the prevention and treatment of gastric ulcers. Given via different routes, it acts on the uterus, causing uterine contractions, cervical dilatation, and an increase in uterine tone. It facilitates tran-cervical procedures such as surgical abortions and hysteroscopy (Bakker et al., 2017). Use of misoprostol has been promoted to ease IUD insertion in nulliparous women (Hall and Kutler, 2016). In women delivered only by elective caesarean sections, the cervix, as seen at transvaginal ultrasound, may +resemble that of a nullipara (Giyahi et al., 2018) Hence, when confronted with such a woman, healthcare staff may be reluctant to insert an IUD because of the uterine scar, and also for fear of encountering difficulties at insertion (Darwish et al., 2018).
520		3	_aبعد الحمل غير المقصود مشكلة عالمية خاصة اذا كانت طريقة الوقاية منة الؤلمة تستخدم الاجهزة داخل الرحم اللولب علي طريقة نطاق واسع كموانع الحمل قابلة العكسي يمثل ادخال اللولب داخل الرحم في النساء اللاتي ولدن فقط عبر عملية قيصرية تجدات فريدة تم استخدام الميزوبرستول وهو نظير البروسياجلانيتن بشكل فعال لايضاح عنق الرحم وتحريض المخاض لة وفؤائدة المحتملة لتسهيل امر ادخال اللولب تستحق البحث هدف هذة الدراسة هو استكشاف الميزويروستول قبل ادخال اللولب الرحمي في النياء اللاتي لدينهن تاريخ من الولادة القيصرية ولم يلدن مهبليا من قبل اجريت تجربة عشوائية مع مشاركةات اللاتي تم تقسيمهم الي ثلاث مجموعات مجموعة تتلقي الميزويروستول تحت السان واخر يتلقي الميزويروستول المهبلي وجموعة مراقبة لا تتلقي اي عاما انضاج عنق الرحم وشملت النتائج الاولية التي تم تقسيمها مدي سهولة الادخال وفشل الادخال و النتائج الثانوية والام المصاحبة لللاجراء و حدوث المضاعفات تشير النتائج الدراسية الي ان استخدام الميزوبرةستول سواء تحت اللسان او عن طريق المهبل يحسن بشكل سهولة ادخال اللولب ويقلل الالام المصاحب للاجراء وتقليل المضاعفات مقارنة بالمجموعة التي لم تتلقي اي علاج ومع ذلك لوحظت ايضا اختلاف بين الطريق تحت اللسان او عن طريق المهبل حيث وجود ان الناتج افضل مع المجموعة التي تلقت العلاج عن طريق المهبل مما ايوفر نظرة ثاقبة اطريقة الادارة الاكثر فاعلية في هذا اليباق يساهم هذا البحث بروي فية حول تطبيق الميزويرستول في اجراء امراض النساء وتحديدا لادخال اللولب الرحمي في النساء اللاتي لدينهن ولادات قيصرية سابقة تدعم الدراسة اعتماد البيرويروستول وخاصة عن طريق المهبل بجرعة 400 مج قبا ادخال اللولب بساعتين كبروتكول قياسي اتحسين نتائج ادخال الللولب في هذة المجموعة من المرضي يوصي باجراء مزيد من من الابحاث لاستكشاف التائثرات طويلة الامد من علاج وتجديد استراتيجتات الجرعات
