520		3	_aBackground: Mastectomy is a cornerstone of disease management for breast cancer. Nurses are in a prime position to prevent postoperative complications, including seroma formation, wound infection, hemorrhage arm pain, stiffness, lymphedema and compromised range of motion of the shoulder and arm. Aim: to evaluate the effect of post mastectomy nursing intervention program on elderly women‟s knowledge, practices and complications‟ prevention. Design: Quasi experimental pre/post one group design was utilized in this study. Setting: The study was conducted in the inpatient and outpatient clinics of the breast cancer hospital, at the National Cancer Institute affiliated to Cairo University.sample:120 elderly women aged 60 years or older who were assigned for mastectomy were included in the study. Tools: 1-Structured Personal and Medical History Questionnaire, 2-Pre /Post Mastectomy Patient's Knowledge Questionnaire 3-The Disabilities of the Arm, Shoulder and Hand (DASH) Questionnaire, 4-Exercises Observational Checklist, 5-Pain Assessment Scale and6- Post Mastectomy Complications Checklist. Results: Age of women in the study sample ranged from 65 to 69 years with mean age of 68.5 and SD = ± 9.2. Regarding to arm, shoulder & hand disabilities, in the pretest there were no disabilities as compared to majority of elderly women who had moderate difficulties at the post & follow-up assessment. "Pain" was the most prevalent complication, followed by "Numbness" and "Restricted Range of Movement in Shoulder". Conversely, some complications demonstrate consistent prevalence rates across both assessments, such as "Lymphedema" and "Hematoma” a highly statistically significant improvement was found in the elderly women regarding their exercise performance and knowledge levels related to breast cancer, mastectomy operation, breast examination in post- program implementation than pre-program with P-value <0.001 **.Conclusion: Application of post mastectomy nursing intervention program had a positive effect on knowledge and practices of elderly women after mastectomy which contributed to complications‟ prevention. Recommendations: Replication of the study on a larger number of breast cancer patients.
520		3	_a تعد عملية استئصال الثدى من اكثر طرق العلاج استخداما لسرطان الثدى. و من هنا جاءت الحاجة الى يرامج التوعية بسرطان الثدى و التى تتضمن معلومات عن اعراض المرض و كيفية التشخيص و طرق العلاج المختلفة و كذلك الممارسات الصحية اللازمة بعد اجراء عملية استئصال الثدى و اخيرا كيفية الوقاية من المضاعفات مثل تورم الذراع ناحية العملية و الالم و عدوى الجرح وتيبث الذراع و اليد و اخيرا الوذمة الليمفاوية. و تهدف الدراسة الحالية لتقييم تاثيربرنامج تداخلى تمريضيى بعد استئصال الثدى على معلومات و ممارسات و الوقاية من المضاعفات لدى النساء المسنات.و تم استخدام تصميم مجموعة واحدة شبه تجريبىة لتحقيق هدف الدراسة. و قد اجريت الدراسة فى مستشفى الثدى التابعة للمعهد القومى للاورام بجامعة القاهرة.و قد اجريت الدراسة على عدد 120 سيدة مسنة من مرضى سرطان الثدى و سيخضعون لعملية استئصال الثدى وتزيد اعمارهم عن 60 عام و تم استخدام ستة ادوات بحثية: (1) البيانات الشخصية و الصحية: (2): قياس معلومات السيدات المسنات قبل و بعد عملية استئصال الثدى (3): مقياس داش الموحد لقياس درجة الاعاقة فى الكتف و الذراع و اليد لدى السيدات المسنات بعد عملية استئصال الثدى . (4): المقياس الرقمى للالم. (5): قياس الممارسات الرياضية للسيدات المسنات. (6): تتضمن اثنى عشر من المضاعفات الاكثر شيوعا بعد عملية استئصال الثدى. النتائج: : اظهرت الدراسة الحالية ان (45.8 %) من النساء المسنات فى المرحلة العمرية من 65- 70 سنة. و كذلك نسبة (65 %) لا يستطيعون القراءة او الكتابة و (89.2%) من السيدات هم ربات بيوت و ايضا اوضحت الدراسة ان نسبة(89.2%) من السيدات ليس لديهم معاش خاص و انما يعتمدون على اسرهم فى الانفاق.و فيما يتعلق بالتاريخ المرضى اظهرت الدراسة الحالية ان (93.3%) لا يعانون من امراض مزمنة.و قد اوضحت الدراسة ان نسبة (97.5%) من السيدات المسنات تحسنت معلوماتهم بعد تعرضهم للبرنامج التمريضى و كذلك اوضحت نتائج الدراسة ان نسبة (78.6%) من السيدات المسنات تحسنت ممارساتهم الصحية الرياضية. و قد وجد ان نسبة (45.8% ) من السيدات المسنات يشعرون باقصى درجات الالم على المقياس الرقمى للالم بعد عملية استئصال الثدى مباشرة و تتحسن هذه النسبة فى مرحلة المتابعة.و اخيرا اظهرت الدراسة الحالية ان كل المضاعفات التى حدثت للسيدات المسنات بعد عملية استئصال الثدى ظلت مستمرة فى مرحلة المتابعة. الخلاصة:يمكن الاستنتاج ان البرنامج التداخلى التمريضى وسيلة فعالة لزيادة المعلومات و الممارسات الصحية و كذلك تقليل المضاعفات بعد عملية استئصال الثدى لدى النساء المسنات توصيات الدراسة:-اجراء المزيد من الابحاث التى تشمل حجم عينة اكبرمن السيدات كبار السن حتى يمكن تعميم النتائج-زيادة حملات التوعية عن مرض سرطان الثدى و كيفية الوقاية من المضاعفات بعد عملية استئصال الثدى -زيادة وعى التمريض و المشاركة فى التثقيف الصحى لمرضى سرطان الثدى.
