520 _aFamilial Mediterranean fever (FMF) is an autosomal recessive auto-inflammatory disease. Recent studies have shown that subclinical inflammation may continue in FMF, even during symptom-free periods. The aim of the current study is to discuss the role of Neutrophil to lymphocyte ratio (NLR) and Platelet to lymphocyte ratio (PLR), as cheap and simple methods, in FMF and whether they can be considered as markers of subclinical activity in FMF children during attack-free periods. One hundred Egyptian children diagnosed as having FMF and in attack-free period, were included in our study together with one hundred healthy children, age and sex- matched. NLR and PLR were calculated at time of study for all participants and compared with demographic, clinical and genetic features of the disease. PLR was significantly higher in children with FMF in attack free period compared to controls (p= 0.009). No significant difference was found between cases and controls regarding NLR. PLR had a significant positive correlation with CRP (p= 0.039) and NLR had a significant positive correlation with ESR (p= 0.023)
