Introduction: Childhood systemic lupus erythematosus (cSLE) is a multisystem disorder with variety of autoantibodies formation. Score of damage index (SDI) can represents the disease effect in multiple systems, but new markers can be used for further assessment and evaluation of disease activity of cSLE. Objective:The present study was conducted to estimate and evaluate Matrix Metalloproteinase-3 (MMP-3) in disease activity of cSLE and its correlation with SDI. Patients and methods:This case control study was conducted on 45 patients with juvenile systemic lupus erythematosus (JSLE) who were diagnosed according to SLICC 2012 criteria (Petri, 2012), and was recruited from the Rheumatology Clinic of children hospital, Cairo University. In addition to 45 healthy age and sex matched control group who were recruited from other clinics at Paediatric Hospital in a period from August 2019 to October 2019. All the included children were subjected to full history taking, complete physical examination, and investigations including; Complete blood picture, serum electrolytes, kidney functions, liver functions, complements, autoantibodies.Disease activity scoring using SDI was done and measurement of MMP-3. Results:There is statistically significant difference regarding level of MMP-3 between patients with different manifestations of SLE throughout course of the disease. There is statistically significant correlation between level of MMP-3 and score of damage index with p value 0.000. Our study revealed statistically significant difference regarding levels of MMP-3 in patients with and patients without neurological manifestations of SLE throughout course of the disease with P-value=0.000
