This study was carried out to compare the clinical success rate of the dental implants that are placed with or without immediate loading in fresh extraction sockets versus healed bony sites. Twenty nine patients (16 females and 13 males with a mean age of 29.03 ± 3.39 years participated in this study and were divided into four groups. The first group (Group A) comprised of seven patients, one patient received 2 implants and the six other patients received a single implant immediately after extraction in fresh extraction sockets with immediate loading. The second group (Group B) comprised of eight patients who received implants immediately after extraction in fresh extraction sockets with delayed loading. The third group (Group C) comprised of six patients two of them received 2 implants and the rest received a single implant in healed bony sites with immediate loading. The fourth group (Group D) comprised of eight patients who received implants in healed bony sites with delayed loading. Immediate loading groups (A and C) received temporary acrylic crowns for 3-4 months which was then replaced with porcelain fused to metal as a final restoration. The patients were evaluated at 3 and 6 month intervals. Clinical criteria were survival rate, bleeding index, probing depth, gingival index and the mobility of the implant (Periotest values). Radiographic evaluation was performed using standardized radiographs to evaluate the surrounding bone at intervals immediately postoperative, 3 and 6 months after the implant placement
