| Summary, etc. | The surgical correction is indicated in rapidly progressive curves, <br/>kyphosis > 70°, respiratory problems, neurological manifestations and the <br/>failure of conservative treatment. <br/>A Ponte osteotomy is the gold standard method for correction of <br/>Scheuermann’s kyphosis. The advantages of the posterior only technique <br/>is being a single stage procedure, with no complications from a <br/>thoracotomy or thoracoscopy and there is no surgical interference with <br/>anterior blood supply to the spinal cord.<br/>Biomechanical data simulating curve correction with variable <br/>implant density suggest that a minimum density screw pattern may result <br/>in a comparable correction as found with a high-density construct.<br/>Fewer pedicle screws may potentially decrease the risk of screw <br/>malposition. Placement of each screw holds a small but significant risk of <br/>malposition, including reported damage to neural structures or viscera.<br/>The main aim of this study was to to evaluate the safety and efficacy <br/>of LSD technique with multiple Ponte osteotomies in correction of <br/>Scheuermann’s kyphosis.<br/> |

| Summary, etc. | التصحيح الجراحي لحداب شيرمان يتم في حالة التقدم السريع للتحدب ، وفي حالة تجاوز درجة الحداب 70 درجة، وفي حالة وجود مشاكل في التنفس، وظهور تظاهرات عصبية، وعدم فاعلية العلاج التحفظي.<br/>إن عملية شق بونتي هي الطريقة الذهبية القياسية لتصحيح حداب شيرمان. تتمثل مزايا التقنية الخلفية فقط في كونها إجراءً على مرحلة واحدة، دون أي مضاعفات في الصدر ، ولا يوجد تدخل جراحي في إمدادات الدم الأمامية إلى الحبل الشوكي.<br/>يتم تقصير العمود الفقري الخلفي بدلا من إطالة العمود الفقري الأمامي، مما يزيد من السلامة. متوسط تصحيح قدره 9.3 درجة لكل عملية شق بونتي . <br/>تم تصحيح 51- 60% من التشوه الحدابي في مرض شيرمان باستخدام التقنية الخلفية الكاملة.<br/>تشير البيانات ان استخدام عدد مسامير اقل في تصليح حداب شيرمان يصل الى نتائج مماثله للتصليح باستخدام عدد مسامير كثيره.<br/>قد يؤدي استخدام عدد أقل من المسامير الفقرية إلى تقليل خطر المخاطر حيث انوضع كل مسمار يحمل مخاطر صغيرة ممكن ينتج عنها الحاق الضرر بالأعصاب او الحبل الشوكي او الاعضاء الداخلية . <br/> |
