Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Oral and Dental Medicine - Department of Prosthodontic Summary: Two implant retained mandibular overdenture is a simple solution in rehabilitation of edentulous patients. Equator attachment offers a lower profile than the self-aligning attachment but there is no enough researches that compare between them. Aim: Clinically compare between both groups regarding oral health related quality of life and overdenture retention in edentulous patients restored with implant overdenture. Material and Methods: Complete denture fabricated, then two implants placed at the canine region. Equator attachments connected to group I and self-aligning attachments connected to each group II. The Arabic version of the oral health impact profile for the edentulous patients was used specifically to evaluate the impact of implant prosthesis on the patient's quality of life. Overdenture retention was measured according to the method reported by (Jose F. Mañes et al., 2011) using a spring scale to measure retention strength (in newton). Results: Regarding oral health related quality of life, there was statistically significant improvement from time of insertion to 3 months in both groups. There was no statistically significant difference between both groups at insertion time and after 3 months. Regarding overdenture retention, there was decrease of retention from time of insertion to 3 months follow-up periods with statistically significant difference in both groups of study. Equator group showed statistically significant more retention force compared to that of locator group at time of insertion, however, there was no statistically significant difference between two groups at 3 months follow-up period
