Aim: Evaluate the effect of MSCs on the healing (regenerative) potentials of the bone in bisphosphonate-induced osteonecrotic teeth socket. Material and methods: A total of twenty-four adult male rats with age ranging from 3 to 4 months and weight ranging from 190-250g were conducted on this study. BRONJ was generated by BPs (Zometa), dexamethozone and mandibular first molar extraction in all rats. After 3 weeks of BRONJ induction, rats were divided into two groups, each consisting 12 rats, the experimental group treated with MSCs topical application and the control group was left untreated. Also, each group was further subdivided into two groups 6 rats for each subgroup a according to sacrifice date at 2 weeks and 4 weeks intervals after MSCs application. The rats were sacrificed by overdose of anesthetic solution (1ml/100gm) according to the research animal guidelines of euthanasia. The mandible of all rats were dissected out and prepared for histopathological examination using (H and E stain), histochemical examination using (masson{u2019}s trichrome) and immunohistochemical analysis, using osteopontin antibody. Results: Revealed formation of new bone in osteonecrotic teeth sockets which are treated with MSCs while progression of BRONJ and no new bone formation in control group. A highly significant difference was detected between MSCs group and control group. Conclusion: MSCs topical treatment approach might help in restoring, maintaining or improving defective tissue functions that have been compromised by BRONJ
