Background: Behcet disease (BD) is a relapsing inflammatory autoimmune disease. MicroRNAs are small RNA fragments that can regulate the gene expression. Autophagy is a protective and life-sustaining process in which cytoplasmic components are targeted to lysosomes for degradation. The aim of this work: To study the association between microRNA199a-5p and ATG5 and Behcet disease activity and severity. Methods: Relative expression of miRNA-199a-5p and ATG5 were measured by Real-Time PCR in blood samples obtained from 47 patients with BD and 50 matched healthy controls. Results: There was a significant higher ESR level in BD patients than controls which highly signifies its autoinflammatory predisposition. Also, there was a significant difference between patients and controls regarding the relative expression of microRNA 199a-5p (p<0.001) with lower expression in patients with BD than in healthy controls and it was significantly lower in patients with thrombophlebitis manifestation. ATG5 relative expression was significantly lower in patients with BD than in healthy controls and ATG5 was significantly higher in behcet patients with arterial thrombosis than other behcet patients
