Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Gynaecology and Obstetrics Summary: The Objective: of our study is to compare the accuracy of ultrasonography (2D and 3D) in prediction of presence of nuchal cord prior to delivery. We also measured the impact of nuchal cord on perinatal outcome. Methodology:150 pregnant women above 37 weeks were examined by 2D and 3D US within 24 hours of delivery. Nuchal cord presence at delivery, mode of delivery and early neonatal outcome were recorded. Results:Nuchal cord incidence was 18%. The sensitivity of 2D ultrasound is 37.04 %compared to that of 3D ultrasound which is 55.56%, where the specificity of 2D and 3D ultrasound was 74.8% and 84% respectively.Out of 53 cases (35.33 %) ended pregnancy by cesarean section, only 13 cases had nuchal cord. The incidence of mechonium staining in all cases was thirty cases (2%). Only five cases had nuchal cord at delivery. 43 neonates had 1-min Apgar score {u2039} 7 at delivery (28.67%). Only eight cases had nuchal cord.Out of those 43 cases, 14 cases continued to have an Apgar score {u2039} 7 even after 5-minutes (3 cases delivered with nuchal cord).Conclusion:. The ability to view the nuchal cord is better with three-dimensional sonography than with two-dimensional. A nuchal cord does not appear to affect the mode of delivery or early neonatal outcome
