Rationale and background: Histone deacetylases (HDAC) are a class of enzymes that allow the histones to wrap the DNA more tightly with the concomitant suppression of gene transcription. HDACs are present in the epidermal layer of the skin, hairs (outer root sheath, and hair matrix), scales, claws and glands. In alopecia areata (AA), Epigenetic factors are involved. The expression of histone deacetylase 1 (HDAC1) was significantly increased with decreased expression of HDAC2 and HDAC7 in peripheral blood mononuclear cells (PBMCs) of patients with AA. These findings prove the involvement of epigenetic factors in the development of the disease. Acne vulgaris has a multifactorial pathogenesis, of which the key factor is genetics. Epigenetic factors have important role in its pathogenesis. Objective: The current study aimed to estimate the serum level of histone deacetylase 1 (HDAC1) in patients with alopecia areata (AA) and acne vulgaris and try to elucidate its possible role in the aetiopathogenesis of both diseases.Patients and methods: This case control study was carried out on 25 alopecia areata patients,26 acne patients & 25 healthy controls. Informed written consents were obtained from all the participants. Patients were subjected to personal history and clinical examination. Photos were taken from patients. Blood samples were collected from the participants to measure the level of HDAC1
