In an attempt to explore genetic factors behind resistance to tamoxifen and if possible targeting these factors or predicting response prior to therapy to avoid Tamoxifen in some patients who are expected to respond poorly to the drug. We prospectively studied the response to tamoxifen in braest cancer patients with measurable locally recurrent and / or metastatic hormone positive disease. We devided the patients into responsive and refractory groups according to response they showed to the drug. We studied the genetic profile of the groups to explore the effect of any genetc aberration that may be responsible for the degree of response to tamoxifen. Results showed that there is a statistically significant correlation between CYP2D6 mutations and resistance to tamoxifen P value = < 0.02
