| Summary, etc. | Placenta accreta spectrum (PAS) is a serious obstetric condition characterized by abnormal placental attachment to the uterine wall. This can occur when the placenta implants too deeply into the myometrium. PAS is classified into three main types: placenta accreta, increta, and percreta. (Jauniaux & Jurkovic, 2012). <br/>The incidence of PAS has been increasing over the past few decades, likely due to the rising rates of cesarean deliveries and other uterine surgeries (Fitzpatrick et al., 2012). <br/>Research significance<br/>PAS is usually managed by cesarean hysterectomy and is associated with high risk for maternal morbidities and blood loss. In a hopeful attempt to lower the blood loss and avoid morbidities, we used a temporary sub-placental uterine tourniquet while performing a bladder last hysterectomy. <br/>Patients and methods<br/>30 patients were enrolled in our study and were randomly divided into 2 groups; a study group in which a temporary uterine tourniquet was applied and a control group where no tourniquet was applied.<br/>Results<br/>Our study showed a statistically significant lower blood loss in the tourniquet group than the non-tourniquet group (Median 957.1 (861.9-1690.2) and 1757.5 (1453-1875.6) respectively with a p value of 0.033). There was also no statistical significance regarding the operative morbidities, the need for blood transfusion, operative time and APGAR score between both groups. <br/>Conclusion<br/>Our study concluded that a temporary sub-placental uterine tourniquet can significantly lower the blood loss during management of PAS cases. It also showed that performing a bladder last cesarean hysterectomy can have a significant impact on the incidence of bladder injury. |
