| Summary, etc. | Purpose: <br/>The objective of this study was to evaluate the clinical and radiographic outcomes <br/>of tibiotalocalcaneal (TTC) nailing in ankle fractures among diabetic patients with <br/>Charcot arthropathy. This technique is particularly beneficial for diabetic patients, <br/>who are prone to complications such as poor bone stock, compromised soft tissue <br/>quality, and delayed healing. <br/>Methods: <br/>This prospective clinical study included 28 diabetic patients with ankle fractures <br/>and Charcot neuropathy who underwent TTC nailing. Primary outcomes assessed <br/>were complication rates and functional outcomes, while secondary outcomes <br/>included the mean time to weight-bearing and union rates. Functional outcomes <br/>were evaluated using the AOFAS (American Orthopedic Foot & Ankle Society) <br/>scoring system. <br/>Results: <br/>At 6 months postoperatively, outcomes based on the AOFAS classification were <br/>excellent in 18 (64.3%) patients, good in 8 (28.6%) patients, and poor in 2 (7.1%) <br/>patients. No statistically significant correlation was found between the final <br/>AOFAS score and factors such as age, BMI, duration of diabetes mellitus, or <br/>HbA1c levels. <br/>Conclusions: <br/>Tibiotalocalcaneal nailing is a reliable and effective treatment option for ankle <br/>fractures in diabetic patients with Charcot neuropathy. It provides favorable <br/>clinical and functional outcomes with a low incidence of complications, making it <br/>a viable choice for ankle fusion in this high-risk population. |
