| Summary, etc. | Background: Cognition consists of several processes, such as learning, <br/>remembering information, organization, planning and problem solving. <br/>(Karabekiroğlu et al., 2005). Chronic rhinosinusitis with obstructive nasal <br/>polyposis (CRSwNP) is known to impact cognitive functions due to persistent <br/>inflammation, nasal obstruction, and associated comorbidities (Tosun et al., <br/>2009) <br/>Objectives: The aim of this study is to evaluate the impact of chronic <br/>rhinosinusitis with nasal polyps (CRSwNP) on cognitive functions and to <br/>investigate the potential improvements in cognition following functional <br/>endoscopic sinus surgery (FESS). <br/>Subjects and methods: This study included 42 patients diagnosed with chronic <br/>rhinosinusitis with nasal polyps (CRSwNP). Each patient underwent <br/>comprehensive assessment both before and after functional endoscopic sinus <br/>surgery (FESS). The evaluation included history taking, otoscopic examination, <br/>basic audiological evaluation, administration of the Nasal Obstruction <br/>Symptom Evaluation (NOSE) survey, completion of the Montreal Cognitive <br/>Assessment (MoCA) questionnaire, and measurement of P300 event-related <br/>potentials. <br/>Results: Most patients exhibited normal middle ear function. Mild hearing loss <br/>was observed in 12%, including sensorineural and conductive types. <br/>Recurrence of nasal polyps occurred in AFRSwNP patients. Cognitive <br/>improvements were significant post-FESS, with P300 latency reduced from <br/>356.89 ms to 333.66 ms (p < 0.001), while P300 amplitude remained <br/>unchanged. Longer disease duration was associated with diminished cognitive <br/>functions (p < 0.001). NOSE score correlated with delayed P300 latency and <br/>impaired MoCA scores pre- and post-FESS. MoCA scores improved from <br/>18.14 to 20.36 (p < 0.001) after surgery. NOSE scores significantly decreased <br/>from 74.64 ± 20.49 to 14.60 ± 14.22 (p < 0.001). Patients with anosmia had <br/>more severe nasal symptoms, poorer cognitive outcomes compared with other <br/>patients <br/>Conclusion: Chronic rhinosinusitis with nasal polyps has an obvious effect on <br/>cognitive functions. FESS significantly improves nasal obstruction, cognitive <br/>function, in chronic rhinosinusitis patients, though longer disease duration and <br/>severe symptoms limit cognitive improvement. |
