| Summary, etc. | Background this study aimed to investigate the effects of pharyngeal <br/>packing on the perioperative gastric residual volume assessed by <br/>ultrasound in patients undergoing endoscopic nasal surgery <br/>Methods This randomized controlled trial was conducted at Cairo <br/>university hospital, Participants were adult patients (above 18 years), <br/>American Society of Anesthesiologists physical status I-II, scheduled for <br/>elective endoscopic nasal surgery under general anesthesia patients were <br/>randomly assigned into two groups using a computer-generated random <br/>number list with closed-sealed envelopes: Group (P): included patients <br/>who received pharyngeal packing after intubation. Group (C): included <br/>patients who did not receive pharyngeal packing after intubation. The <br/>primary outcome of the current study was GRV at the end of the <br/>procedure. <br/>Results <br/>The primary outcome of the current study is GRV at the end of the <br/>procedure. <br/>In this non-inferiority analysis assessing gastric residual volume (GRV) <br/>in patients undergoing endoscopic nasal surgery, the non- packing group <br/>demonstrated a statistically significant reduction in mean GRV compared <br/>to the packing group. The observed mean difference in GRV was –12.35 <br/>mL (non-packing - packing), with a 95% confidence interval (CI) ranging <br/>from –19.72 to –4.97 mL. Since the entire CI lies below the pre-specified <br/>non-inferiority margin of 10 mL, the null hypothesis of inferiority was <br/>rejected, confirming non-inferiority of the non-packing group. <br/>Furthermore, as the CI does not include zero and the p-value was < p < <br/>0.001, superiority of the non- packing group over the packing group in <br/>terms of GRV can also be inferred. <br/>Conclusion: These findings indicate that omitting pharyngeal packing is <br/>statistically non-inferior, and potentially superior to using pharyngeal <br/>packing regarding perioperative gastric residual volume. This could <br/>indicate that avoiding pharyngeal packing might offer a comparable level <br/>of perioperative safety, particularly regarding gastric content <br/>accumulation, without introducing additional risks |
