| Summary, etc. | Background <br/>Late-presenting fractures of the lateral humeral condyle (LHC) are misdiagnosed or insufficiently <br/>treated fractures, presenting later than 3 weeks after injury. The management of Late-presenting LHC <br/>fractures in children remains controversial. <br/>Materials and methods <br/>A prospective case series study which included 30 patients, 21 males and 9 females, with mean <br/>age at diagnosis: 8.6 years. Baseline radiographs were assessed for fracture grading and displacement. <br/>Final radiographs were investigated for bone healing, avascular necrosis, elbow deformities and growth <br/>disturbances. Outcome analysis included standard clinical, including functional scores (Dhillon score and <br/>Mayo Elbow Performance Score), in addition to plain radiographs. The management plan was revised at <br/>the end of the study. <br/><br/>Results <br/><br/>The mean time from injury to presentation was 35.42 months. Treatments included nonoperative <br/>management (4 patients), “in-situ” fixation (2 patients), open reduction and internal fixation (11 patients). <br/>A subset of those (2) were presenting later than 1 year (range 1-6.25 years), corrective osteotomy (11 <br/>patients) and excision of bony spurr (2 patients). The mean follow-up was 25.06 months (range: 12-75). <br/>Overall, we observed complications in 2 patients (6.6%). One patient had a recurrent cubitus varus <br/>deformity necessitating rev, and another had a limitation of range of motion. At the latest follow-up, pain <br/>and functional scores improved in 29 of 30 patients (96.6%) compared with the baseline. ROM arc <br/>increased on average by 20 degrees (p = 0.01). Pronation and supination were normal in all patients. <br/>MEPS increased on average from 60 points at baseline to 95 points at the latest follow-up visit (p < <br/>0.001), while Dhillon score improved on average from 4.83 to 7.23 points (p < 0.001). <br/><br/>Conclusion <br/>Our study describes outcomes from a cohort of children undergoing different treatment modalities <br/>for late-presenting LHC fractures. Our findings establish the need for standardized surgical management <br/>in these late-presenting fractures, for which we propose clear, evidence-based guidelines. ORIF remains a <br/>viable option even in extremely late-presenting LHC fractures when stable fixation can be achieved. <br/>Rigid fixation with small fragment set is increasingly needed. |
