| Summary, etc. | Background: Children undergoing hemodialysis are exposed to frequent arteriovenous fistula <br/>(AVF) puncture, often leading to significant pain and behavioral distress, affecting children's <br/>well-being and treatment adherence. Virtual reality (VR) has emerged as promising non- <br/>pharmacological nursing interventions to decrease pain intensity and behavioral distress for <br/>children. Aim: The current study was to evaluate the effect of VR on pain intensity and <br/>behavioral distress during AVF puncture among children undergoing hemodialysis. Research <br/>design: A quasi–experimental research design (one group pre-posttest) design was utilized. <br/>Sample: A convenient sample of 30 school age children undergoing hemodialysis with AVF. <br/>Setting: The study was carried out in Cairo University Children Hospital at El- Mounira section <br/>of pediatric nephrology and transplantation. Tools: Three tools were used: (1) Structured <br/>interview questionnaire; (2) Pain assessment tool (Wong–Baker faces pain rating scale); (3) The <br/>Procedure Behavior Checklist Scale. Results: The current study revealed that mean age of <br/>children was 9.80 ± 2.16 years, during AVF puncture which is the most prominent phase of pain <br/>intensity and behavioral distress, pain intensity pre intervention (day1) more than half of <br/>children had severe pain while post intervention (day2,3) highest percentage of children had <br/>moderate pain with application of VR intervention. The mean scores of behavioral distress pre <br/>intervention (day1) were 23.77 ± 8.15, which reduced post intervention (day 2,3) to (16.53 ± <br/>7.70; 13.77 ± 7.61 respectively) with application of VR intervention. There was a highly positive <br/>correlation between pain intensity and behavioral distress pre intervention (day1) and post <br/>intervention (day 2,3). Conclusions: Virtual reality is effective in reducing AVF puncture pain <br/>intensity and behavioral distress for children undergoing hemodialysis. Recommendations: <br/>Integrate VR into children’s hemodialysis care to reduce pain and behavioral distress during <br/>AVF puncture. |

| Summary, etc. | الخلفية العلميه: يتعرض الأطفال الذين يخضعون لغسيل الكلى الدموي لثقب الناسور الشرياني الوريدي بشكل متكرر، مما يؤدي غالبًا إلى ألم شديد واضطرابات سلوكية، مما يؤثر على صحة الأطفال والالتزام بالعلاج. برز الواقع الافتراضي كتدخلات تمريضية غير دوائية واعدة لتخفيف شدة الألم والضائقة السلوكية لدى الأطفال. الهدف: هدفت الدراسة الحالية إلى تقييم تأثير الواقع الافتراضي على شدة الألم والاضطراب السلوكي أثناء ثقب الوصلة الوريدية الشريانية لدى الأطفال الخاضعين لغسيل الكلى. تصميم البحث: استُخدم تصميم بحث شبه تجريبي (مجموعة واحدة قبل وبعد الاختبار). العينة: عينة ملائمة من 30 طفلًا في سن المدرسة يخضعون لغسيل الكلى مع وجود ناثور وريدي شرياني. مكان الدراسة: أُجريت الدراسة في مستشفى الأطفال بجامعة القاهرة، قسم أمراض وزراعة الكلى للأطفال بالمنيرة. الأدوات: تم استخدام ثلاث أدوات: (1) استبيان المقابلة المنظمة؛ (2) أداة تقييم الألم (مقياس وونغ-بيكر لتقييم آلام الوجه)؛ (3) قائمة ملاحظات لتقييم سلوك الإجراء. النتائج: كشفت الدراسة الحالية أن متوسط عمر الأطفال كان 9.80 ± 2.16 سنة، خلال ثقب الوصلة الوريدية الشريانية وهي المرحلة الأكثر بروزًا من حيث شدة الألم والاضطراب السلوكي، وشدة الألم قبل التدخل (اليوم الأول) كان لدى أكثر من نصف الأطفال ألم شديد بينما بعد التدخل (اليوم الثاني والثالث) كانت أعلى نسبة من الأطفال يعانون من ألم معتدل مع تطبيق تدخل الواقع الافتراضي. بلغ متوسط درجات الاضطراب السلوكي قبل التدخل (اليوم الأول) 23.77 ± 8.15، وانخفض بعد التدخل (اليوم الثاني والثالث) إلى (16.53 ± 7.70؛ 13.77 ± 7.61 على التوالي) مع تطبيق تدخل الواقع الافتراضي. وُجد ارتباط إيجابي كبير بين شدة الألم والاضطراب السلوكي قبل التدخل (اليوم الأول) وبعده (اليوم الثاني والثالث). الاستنتاجات: الواقع الافتراضي فعال في تخفيف شدة ألم ثقب الوصلة الوريدية الشريانية والاضطراب السلوكي لدى الأطفال الخاضعين لغسيل الكلى. التوصيات: دمج الواقع الافتراضي في رعاية غسيل الكلى للأطفال لتقليل الألم والاضطراب السلوكي أثناء ثقب الوصلة الوريدية الشريانية. |
