Both anterior-to-posterior screw fixation using indirect reduction and posterior
buttress plating via direct reduction through the posterolateral approach are viable
options for stabilizing the posterior column in Pilon fractures involving two, three, or
four columns, and both can result in successful fracture union. However, based on our
findings, posterior plating provided superior reduction quality and yielded better
overall functional outcomes as reflected by higher AOFAS scores.
