Milrinone is a cyanopyridone derivative that has been introduced for the treatment of CHF . Its mechanism of action involves inhibition of PDE3 leading to elevation of cAMP increasing the entry of Ca⁺⁺ ions into the cardiac tissues and hence increasing the contractility of the cardiac muscle
